Steps to Success:
1) Patient Referral
Patients are referred for physical therapy by obtaining a written prescription from their physician, dentist or podiatrist. We maintain open and frequent communication with the referring doctors to update them on the course of treatment and progress of each patient.
2) Patient Preparation
Patients will be required to enter themselves into our system by one of two methods:
1) Electronically fill out their forms below, print them off at home, and bring them to their first appointment.
2) Fill their information out by hand prior to their first appointment.

3) Evaluation and Treatment
Our physical therapists (PT) look for the source of the problem by using special manual therapy technique to test muscles, individual spinal segments, and extremity joints. The PT uses tests and to provoke and alleviate a patient’s symptoms to best identify the source of the problem. The PT evaluation consists of measurable, objective, and functional findings including range-of-motion, neurological screening, strength, endurance coordination and flexibility.
A patient’s individualized OMPT program includes:
- Patient education
- Manual traction, joint and soft tissue mobilization
- Functional stretching
- Spinal and extremity dynamic stabilization
- Unweighted exercise
- Biomechanical training and sports medicine
- Individualized therapeutic exercise
- Ergonomic evaluation of work and lifestyle
4) What to Expect from Treatment
As a patient of Midland Physical Therapy, Inc., you can expect to work closely with your physical therapist in a cooperative effort to accomplish your treatment and recovery goals.
Your first appointment is normally scheduled for one hour with the PT. Before determining how to treat your problem, the PT will conduct a full biomechanical evaluation. Specific manual therapy techniques are utilized to identify the source of your symptoms. Your movement, strength, reflexes, sensation, gait, and balance will be measured. You will be asked about your symptoms, history, and any activities that may contribute to your condition. This is a good time to bring up any questions or concerns you may have.
Treatment will consist of manual techniques and appropriate exercises to improve mobility and reduce pain. Based on your evaluation, a specifically tailored program will be developed and conducted under the close supervision of your physical therapist. Selecting from a wide range of treatments and literally thousands of exercises, the therapist designs a custom program to address each patient’s specific problems. Our gym facility is highly adaptable to meet all of your specific rehab needs.
Patient cooperation is vital to successful recovery. Each individual treatment plan balances not only such physical factors as muscles, joints, and body types, but also the realities of each patient’s lifestyle, schedule, activities, and commitment level. You will learn about your condition and be instructed in ways to relieve pain and reduce the risk of further injury.
