Physical therapy is a normal service generally covered by most insurance plans. The patient is responsible for contacting his or her insurance carrier to determine physical therapy benefits.

Our office will submit claims directly to the patient’s insurance company. Midland Physical Therapy, Inc. is a participating provider for:

Our office also directly bills most traditional indemnity insurance plans.
